
Our Life Care Plan (LCP) service is a specialized offering addressing the needs of those who have been catastrophically injured or have a chronic health condition. The experts at TPLCP are occupational therapists (OT) and a nurse practitioner (NP), who are certified life care planners, each bringing extensive clinical expertise that is applied to each life care plan. After a thorough review of the medical records, a comprehensive assessment is conducted within the expert’s own scope of practice.
Occupational Therapists are qualified to assess the physical, cognitive and emotional impairments that are restricting function while considering the physical and social environment, which may facilitate functioning or pose a barrier. Occupational therapists opine on functional capacity and the need for evaluations, rehabilitation, durable medical equipment and assistive devices, home and environmental modifications, adaptive equipment and technology, community and vocational needs, and long-term support, e.g. attendant care, homemaking, etc.
A Nurse Practitioner clinical assessment may include symptom review, medication and treatment analysis, and a physical assessment to opine on a medical and nursing diagnoses and future medical needs such as medical monitoring, medications, diagnostic testing, therapeutic interventions, durable medical equipment and medical supplies, and nursing and caregiver services.
As certified life care planners, we collaborate with other healthcare providers and/or retained experts, determine the standards of care, extensively research appropriate and reasonably available care options for the client, and analyze the data to identify the client’s specific needs. This collaborative approach strengthens the medical foundation of the Life Care Plan, ensuring a well-rounded and expertly crafted assessment.
